  • We offer on-site cardiology services that are proven to reduce Emergency Department visits and readmissions. We assist in the management of various cardiac conditions commonly seen in the post-acute setting. Though we do treat acute illness, we view ourselves as a preventative physician group, catching the disease process early and treating it as indicated. Through our transitional care program and hospital relationships, we ensure that the patient admits and discharges successfully. Our proven clinical outcomes are a result of incorporating cutting-edge technology to aid in diagnosing and treating patients, interpreting EKG results, providing educational resources for our staff and educating the facility staff on cardiology best practices. Our services include cardiology care, dietary counseling, fluid restriction guidance and personalized medication education.

  • Our team works to prevent, diagnose and treat a wide variety of renal diseases. Our on-site services include fluid and electrolyte management, evaluation of frequent UTIs, kidney stone management, anemia management, refractory hypertension management, CKD optimization and dialysis care.

  • We ensure proper management of a patient's health condition through various logistical arrangements, educating the patient and their family and coordinating with their cardiology and nephrology groups. For the best possible outcomes, we work with patients and their outpatient cardiologist or nephrologist after treatment for conditions like sleep apnea, COPD, pneumonia and more.

  • CRV is partnered with RenViva, a national provider of dialysis services with 60+ years of combined experience and is involved in developing over 170 dialysis facilities. RenViva provides in-house dialysis therapies in skilled nursing and post-acute care, improving convenience for patients and staff. Our experienced renal nurses and patient care technicians improve communication with the skilled nursing personnel to achieve better patient outcomes. We use state-of-the-art equipment and EMR systems that on-site caregivers can access and report monthly quality results to ensure patients meet CMS standards.
